These forms will be available at the front desk as well as in your recital guide, but feel free to print out what you need and turn them in to the front desk.

DVDS: You get your first DVD FREE. Additional orders are $15 each and you can print out an order form HERE.

T-SHIRTS: These are included with your recital fee.

RECITAL ADS: These may also be purchased. Forms are due May 25 and you can print your order form HERE.

FLOWERS: If you'd like a recital t-shirt, you can print out an order form HERE.
